- Submit filled-up Business Permit Application Form (BAPF) with complete requirements attached.

- Anyone of the following: Jennifer Orcullo, Edsel Saligumba and Ian Arig

- Receives & reviews BAPF for completeness of data and attachments
- Prepares tax due worksheet &,
- Prepares tax due worksheet &,
- Prepares tax due worksheet &,
- Advise the client to go to City Treasurer’s Office for payment of fees and charges

- Presents the official receipts (ORs) issued by the City Treasury.

- Anyone of the following: Jennifer Orcullo, Edsel Saligumba and Ian Arig

- Checks the ORs & validates the payment made thru the computer, and
- Prints the Business Permit for approval & signature of the City Mayor or his duly authorized representatives:

- Receives the approved Business Permit & affixes his/her signature therein, agreeing thus to the terms & conditions as stipulated in the Business Permit.
- Returns the signed Business Permit for proper release & entry in the office logbook.

- Segregates documents (copy for the client and copy for office file),
- Have the client sign in the “Approved Business Permits” logbook, and
- Releases to the client the approved Business Permit, BAPF; also the Business Plate or sticker, as appropriate.

Requirements for Renewal of Business Permits
A. Renewal
- Accomplished Business Permit Application Form
- Barangay Clearance to operate the business from where the business establishment is located
- Barangay Resolution, if operating a videoke, karaoke, or any other machines for amusement or gaming
- Community Tax Certificate
- Previous Year’s Business Permit
- City Treasurer’s Office (Real Property & Business Tax Clearance)
- If Tax Exempt: Certificate of Tax Exemption
- Financial Statement of BIR Annual Income Tax Returns
- If processing is done by a representative:
- For Single Proprietorship: Special Power of Attorney
- For Corporation: Secretary’s Certificate
- For Cooperative: Secretary’s Certificate
Fees and Charges: Depend on the line of business per City Revenue Code, as amended

B. New Establishments
- Accomplished Business Permit Application Form
- Barangay Clearance to operate the business from where the new business establishment is located
- Barangay Resolution if operating a videoke, karaoke or any other machines for amusement or gaming
- Community Tax Certificate
- Location/Vicinity Map
- Proof of Capitalization
- Sworn Statement
- Tax Declaration of the real property where the new business is located
- One (1) passport size picture of the owner, operator, president or general manager

- City Health Office
- City Planning & Development Office
- City Environment & Natural Resources Office
- City Engineer’s Office/Building Official
- City Treasurer’s Office (Real Property Tax Clearance)
- Philippine National Police (Police Clearance)
- Bureau of Fire Protection
- (If Tax Exempt) Certificate of Tax Exemption
· Other Requirements:
- Tax Identification Number
- For Corporation: SEC Registration, Articles of Incorporation & By-Laws
- For Cooperative: CDA Registration, Articles of Incorporation & By-Laws
- For General Engineering Business: Contractor’s License
- For Selling of Drugs & Related business; also Food Processing like Bakery: BFAD License to Operate
- For Beach Resort: Environmental Compliance Certificate and Foreshore Lease Agreement
- For Banks & Pawnshops: Central Bank Accreditation
- For Schools: DECS Accreditation
- For Retailer/Dealer of Lumber (Miscellaneous Trees): DENR Clearance
- For Retailer/Dealer of Coco Lumber: PCA Registration
- For Public Market and Other Economic Enterprise Stallholders: Contract of Lease
- For Tourism Related Establishments: City Tourism Office Accreditation
- For Livestock Related Activities: CVET Clearance
- For Aquaculture Related Activities: CFARMO Certification/Clearance
- For Agriculture Related Business: CAGRO Certification/Clearance
